It is called N°4 because it resulted from the fourth stage of poppy sap refinement. In other words it showed up after opium, morphine and brown sugar. Because of the combination of unemployment and easy access to heroine, coming directly from the Myanmar side of the Golden Triangle, the North East of India has one of the highest HIV positive rates among drug users and sex workers, considering the 8% positivity rate overall the Sub Continent.
